This guideline provides valuable information for instructors about testing procedures and types of testing accommodations. These guidelines are valid on all campuses.

Documentation
If you were in special education in high school and have documentation to support your disability, use this form to request information from your previous educational institution.
If you have a medical condition, use this form to request information from your Medical doctor or specialist. If students or instructors have any questions about testing accommodations, please contact the Disabilities Assistant/Technician at your campus.
