Request For Testing Accommodation Form
There are two steps that students must take for every test, exam, quiz or in-class essay written in the CSD:
First, students must book the time, space and any technology needed with the Disabilities Assistant/Technician (CSD Testing Coordinator) at least one (1) week prior to the requested test date.
Second, they must fill out the top portion of the Request for Testing Accommodation Form, check off the accommodations required, and give this form to instructors for each test being written. Students should remind the instructor to: fill in the section marked Faculty; initial appropriate accommodations; and bring the form with the test to the CSD. If any preparation is required (for example, scanning or enlarging the test), students should remind the instructor that the CSD needs the test two days before the test date.

    This guideline presents a detailed explanation of testing procedures if students are writing tests etc. in the CSD, as well as general information on testing accommodations. These guidelines are valid on all campuses. It is the student's responsibility to be aware of these procedures and guidelines.
  • This guideline provides valuable information for instructors about testing procedures and types of testing accommodations. These guidelines are valid on all campuses.

Documentation

If you were in special education in high school and have documentation to support your disability, use this form to request information from your previous educational institution.

If you have a medical condition, use this form to request information from your Medical doctor or specialist.

If students or instructors have any questions about testing accommodations, please contact the Disabilities Assistant/Technician at your campus.

 

Alternative Media Request Form
There are two steps that students must take to request books in alternative from the CSD:
First, students must download and fill in the Alternative Media Request Form.
Second, they must bring in the Alternative Media Request form along with proof of ownership of the requested book and the book itself to their campuses Disabilities Assistant/Technician.

Procedure for using the forms on our website
