Diane MacEachern

NP-PHC, MScN

Hi! My name is Diane MacEachern and I have been nursing for almost 29 years - 23 years as a Nurse Practitioner. I spent the first 17 years of my career working in public health/primary care settings caring for both youth and young adults – during that time I had the privilege of working within a terrific interdisciplinary health care team that focused on health and wellness and supported young people in managing a variety issues including those related to sexual health, mental health and pre/post-natal health. The past 12 years of my practice has focused on the primary care of the frail elderly. I have a keen interest in nursing care that supports those whom are living with dementia. Supporting residents and families through their palliative and end-of-life journeys is also very important to me.

I am a lifelong learner and have always enjoyed a role in supporting other nurses and students in this regard. I have taught and preceptored at Ryerson University in the NP program and have been both a Clinical Instructor and Faculty Advisor in the School of Nursing at UOIT/Ontario Tech. I joined the team here at Centennial College last spring in a part-time contract capacity and have been full-time in the Collaborative BScN and Bridging to University Programs since January 2020.
