Documentation for First Appointment

Students who are accessing CALCS for the first time for disability and accommodation related reasons are asked to prepare soft copies of their available supporting documentation (see below):

 

Type of Disability Examples of Supporting Documentation
  • Learning Disability
  • Specific Learning Disorder
  • Mild Intellectual Delay
  • Autism Spectrum Disorder
  • Acquired Brain Injury
  • Psycho-educational assessment(s)
  • Most recent Individual Education Plan (IEP)
  • Accommodation Letters from other post-secondary institutions
  • ADHD/ADD
  • Psychiatric Diagnoses (e.g., depression, anxiety, bipolar disorder, etc.)
  • Chronic Medical Conditions
  • Mobility or Functional Limitations
  • Low vision or Blind
  • Deaf, Deafened or Hard of Hearing

How to send your supporting documentation to CALCS:

  • In advance of your appointment, you may e-mail CALCS at calcs@centennialcollege.ca or send a virtual fax to 647-689-2932.  Be sure to include your name and student number in the body of the email.
  • If you are meeting with one of our Counsellors in person, you can bring in copies of your documentation
  • The validity of psycho-educational assessments is 5 years if under 17 years old or lifetime if completed over 18 years old.
